I am in network with several insurance companies. For health insurance plans I bill directly, sometimes there is cost sharing, such as a co-pay, deductible, or co-insurance. Most plans cover lactation services under preventative care at 100%. I can offer a superbill to self-pay patients in order to submit for reimbursement.

*There is a $75 travel fee for home visits within a 20 mile radius of 91362. Outside this radius there is a $150 fee. Please note that this fee is not covered by insurance.
